Memo to Parents/Guardians

H1N1 School Immunization – Grades 7 - 9

 

Eastern Health will begin vaccinating students in grades 7 - 9 at Holy Redeemer Wednesday Morning starting at 9 a.m.

If you have already submitted a vaccination consent form, and have since taken your child to be immunized, it is critical that you inform your child’s classroom teacher immediately.

If you want your child to be vaccinated at school, please ensure the vaccination consent form is fully completed, signed and returned to your child’s classroom teacher.  Forms are available by clicking on the link below, at the school, or from the District website: www.esdnl.ca.

If a parent or guardian wishes to accompany their child to the school immunization clinic, please check in at the school office.

Note: Parents/guardians wishing to be present during their child's vaccination, please proceed to the cafeteria and wait to be notified.

Important Message to Parents Regarding In-School H1N1 Immunization Program October 30, 2009

Eastern Health is working on a plan to immunize all school-aged children in the school setting wherever possible and feasible. To have children vaccinated at school, parental consent is required.

If you wish to have your child receive the H1N1 vaccination at school, please complete the parent consent form and return it to your child's classroom teacher as soon as possible.

Further details on the schedule for school vaccinations will be provided next week.
